ERISA Wrap documents
An ERISA Wrap is one welfare plan document with a summary plan description (SPD) that brings your fully insured benefits together under a single plan.
What you receive
A Wrap Package is planned to include the wrap plan document and SPD, a schedule of your insured benefits, your Carrier Documents as exhibits, a Company Action, and Employer instructions. The final list is pending counsel approval.
Benefits we can include
Each must be fully insured, meaning an insurer pays claims:
- Medical
- Dental
- Vision
- Group term life
- Accidental death and dismemberment (AD&D)
- Short-term and long-term disability
- Insured employee assistance program (EAP)
Benefits we cannot include
- Self-funded or level-funded coverage
- Health reimbursement arrangements (HRAs)
- Health flexible spending accounts (FSAs)
- Health savings accounts (HSAs) as a benefit
- Commuter benefits
- Voluntary or individual policies
Not sure how a benefit is funded? You can say so in the questions; we do not guess.
What you will need
The certificate, booklet or policy from your insurer for each benefit. These are your Carrier Documents, and you upload them during intake.
